Is This Right for You?

Over 45 and Tired of Squinting at Your Phone?

After 45, the eye's natural lens stiffens — making glasses for near and often distance an unavoidable nuisance. Custom Lens Replacement addresses both.

You may be a candidate if you…

  • Reach for glasses — reading, distance, or both — multiple times a day
  • Hold menus, phones, or books at arm's length
  • Wear bifocals or progressives and hate them
  • Have early cataracts or been told LASIK isn't ideal
  • Want a long-lasting vision correction with lasting results
  • Are in generally good health with no active eye disease
  • Want to be glasses-free for golf, tennis, pickleball, boating, or travel

Why Custom Lens Replacement Works Long-Term

Unlike LASIK — which reshapes the cornea but can't fix presbyopia — Custom Lens Replacement (also called Clear Lens Extraction or Refractive Lens Exchange) removes your eye's aging natural lens entirely and replaces it with a premium intraocular lens (IOL) engineered for crisp vision across a wide range of distances.

The implanted lens is a stable, long-lasting solution. There's no regression in the lens itself. Once it's in place, it remains for life — and you will also never develop cataracts in that eye.

Key difference from LASIK: CLR addresses both presbyopia (reading vision) and distance simultaneously — something LASIK cannot achieve in patients over 45.

Additional Options
Bausch + Lomb
enVista Aspire & Aspire Toric
Trifocal · Full Range of Vision

A strong choice for patients who want excellent all-distance vision with fewer night-time visual disturbances. The Aspire Toric features the widest range of astigmatism correction built right into the lens.

  • Widest astigmatism correction range of any toric premium lens
  • No extra laser step needed — correction is built in
  • Fewer halos and glare vs. traditional multifocals
  • Lower cost than comparable toric premium options
Additional Options
J&J Vision · Alcon
PureSee & Vivity
Extended Range · Good for Complex Eyes

TECNIS PureSee may be the right choice for patients whose eyes aren't ideal candidates for a full trifocal — those with glaucoma, macular degeneration, or other conditions. Vivity's non-diffractive technology offers minimal halos, well-suited for frequent night drivers.

  • PureSee: good option for eyes with glaucoma or macular degeneration
  • Reliable distance and intermediate vision without glasses
  • Reading glasses may still be needed for near tasks
  • Vivity: minimal halos — excellent for night-time driving

As a retired attorney from South Tampa, I did months of research before choosing Dr. Mahootchi. After my Aspire Toric procedure I can read a menu, see a face across a restaurant, and drive at night — all without glasses.

I was terrified of eye surgery. The staff walked me through every single step. Surgery took maybe 12 minutes and I felt nothing. Next morning I could see my alarm clock, read my phone, and see our lakehouse across the water — all without glasses.

Three other ophthalmologists told me I wasn't a great LASIK candidate. Dr. Mahootchi immediately recognized I was a perfect CLR candidate instead. Now at 52 I have 20/20 vision and read without glasses for the first time in 15 years.

After years of progressives that I hated, my CLR with PanOptix changed my life. I'm 61, I sail competitively out of Tampa Bay, and I can now read charts, see the horizon, and read my instruments without a single pair of glasses on the boat.

Frequently Asked Questions

Everything You Need to Know Before You Decide

What exactly is Custom Lens Replacement?
Custom Lens Replacement (CLR) — also called Clear Lens Extraction (CLE) or Refractive Lens Exchange (RLE) — is a surgical procedure where your eye's natural lens is removed and replaced with a premium intraocular lens (IOL). Unlike LASIK which reshapes the cornea, CLR addresses the root cause of presbyopia by replacing the stiffened lens. The result is permanently clear vision at all distances.
How is this different from cataract surgery?
The surgical technique is essentially identical — both remove the natural lens and replace it with an IOL. The key difference is timing and IOL selection. CLR is performed electively before cataracts develop, using premium presbyopia-correcting IOLs to achieve spectacle independence. A major bonus: once your natural lens is removed, you will never develop cataracts.
Am I too old or too young for this procedure?
CLR is ideal for adults generally between 45 and 75. Patients must have stable vision and healthy eyes without active retinal disease or severe dry eye. There is no firm upper age limit — many patients in their 70s are excellent candidates. We will assess your specific eye health and prescription during your consultation.
Will I significantly reduce my glasses use after CLR?
The majority of patients who receive a trifocal IOL such as PanOptix Pro or enVista Envy report they rarely or never need glasses in daily life. Clinical studies consistently show high rates of spectacle independence across all distances. Dr. Mahootchi will set honest, realistic expectations based on your specific eyes during your consultation.
How much does Custom Lens Replacement cost in Tampa?
Our pricing is 30–50% below most Tampa Bay competitors. Premium trifocal lenses start around $2,800 per eye (compared to $4,500+ elsewhere), with all follow-up visits included. Exact pricing depends on your lens selection and any astigmatism correction needed. Flexible financing options are available.
Does insurance cover Custom Lens Replacement?
Standard CLR (elective, non-cataract) is not covered by health insurance or Medicare as the surgery itself is elective. However, if a cataract diagnosis exists, the basic cataract surgery portion may be covered — the premium lens upgrade would be an out-of-pocket cost. Our billing team will review your specific insurance situation during your consultation.
What is the recovery like? Will I need time off work?
Most patients notice dramatically improved vision within 24–48 hours. Mild blurriness or light sensitivity is normal for a few days. Most patients return to desk work within 1–3 days. Physical activity restrictions apply for about 2 weeks. We typically treat one eye at a time, scheduled 1–2 weeks apart.
Why choose Dr. Mahootchi vs. other Tampa eye surgeons?
Dr. Mahootchi has performed over 25,000 lens procedures in the Tampa Bay area with nearly 30 years of experience since 1997. He offers the complete portfolio of premium IOLs (most practices stock only 1–2), takes the time to personally explain every option, and charges significantly less than corporate eye care chains. His 4.9-star rating across 1,011+ certified reviews speaks for itself.
